Na prvi pogled, ove pojave mogu delovati nepovezano. Jedna se odnosi na disanje, druga na dr\u017eanje tela, tre\u0107a na ravnote\u017eu ili pokret. Me\u0111utim, telo ne funkcioni\u0161e kao skup odvojenih delova. Kod dece, posebno kod dece sa motori\u010dkim ili razvojnim pote\u0161ko\u0107ama, na\u010din na koji je organizovan grudni ko\u0161 mo\u017ee imati va\u017enu ulogu u tome kako dete di\u0161e, sedi, stoji, koristi ruke i odr\u017eava stabilnost. Grudni ko\u0161 nije va\u017ean samo zato \u0161to \u0161titi plu\u0107a i srce. On je jedna od centralnih struktura preko koje telo povezuje disanje, posturu i pokret. Kada grudni ko\u0161 pru\u017ea dobru podr\u0161ku, telo lak\u0161e raspore\u0111uje optere\u0107enje, odr\u017eava polo\u017eaj i koristi energiju za funkcionalne aktivnosti. To je prirodno, jer se kroz pokrete grudnog ko\u0161a odvija disanje. Ipak, njegova uloga je mnogo \u0161ira.<br \/>Grudni ko\u0161 je povezan sa ki\u010dmom, ramenima, vratom, dijafragmom, trbu\u0161nim mi\u0161i\u0107ima, karlicom i fascijalnim strukturama koje povezuju razli\u010dite delove tela. Kada dete izdahne, telo ponovo organizuje pritisak i tonus. Taj ritam se de\u0161ava stalno, \u010dak i kada dete miruje.<br \/>Drugim re\u010dima, <b>disanje nije odvojeno od posture<\/b>. Postura je na\u010din na koji telo organizuje svoje delove u odnosu na gravitaciju, oslonac i pokret. Ona omogu\u0107ava detetu da sedi, stoji, gleda, okre\u0107e se, koristi ruke, di\u0161e i reaguje na promene polo\u017eaja.<br \/>Grudni ko\u0161 u tome ima va\u017enu ulogu jer se nalazi izme\u0111u glave i karlice. On je kao centralni deo tela preko kojeg se prenose informacije i optere\u0107enja izme\u0111u gornjeg i donjeg dela tela.<br \/>Ako je grudni ko\u0161 previ\u0161e uko\u010den, spu\u0161ten, rotiran, asimetri\u010dan ili stalno u polo\u017eaju napetosti, ostatak tela mora da se prilago\u0111ava. Glava mo\u017ee i\u0107i unapred. Ramena mogu biti podignuta. Vrat mo\u017ee postati napet. Karlica mo\u017ee promeniti polo\u017eaj. Ruke mogu izgubiti stabilnu osnovu za pokret. Hod mo\u017ee izgledati manje organizovano.<br \/>Roditelj tada \u010desto vidi samo posledicu: dete se krivi, te\u0161ko sedi, \u010dudno hoda, brzo se umara ili deluje uko\u010deno. Ali uzrok nije uvek tamo gde se posledica najvi\u0161e vidi.<br \/><br \/><strong>Grudni ko\u0161 i polo\u017eaj glave<\/strong><br \/>Polo\u017eaj glave je jedan od prvih signala koje roditelji prime\u0107uju. Dete mo\u017ee stalno dr\u017eati glavu unapred, naginjati je na jednu stranu ili te\u0161ko odr\u017eavati stabilan polo\u017eaj glave tokom sedenja i igre.<br \/>Iako deluje kao da je problem samo u vratu, glava retko funkcioni\u0161e odvojeno od ostatka tela. Da bi glava bila stabilna, potrebna joj je podr\u0161ka iz trupa. A grudni ko\u0161 je va\u017ean deo te podr\u0161ke.<br \/>Ako grudni ko\u0161 ne pru\u017ea dovoljno stabilnu osnovu, vrat \u010desto mora da radi vi\u0161e. Dete tada mo\u017ee izgledati kao da stalno \u201cdr\u017ei\u201d glavu mi\u0161i\u0107ima vrata. To mo\u017ee biti naporno i mo\u017ee uticati na pa\u017enju, pogled, hranjenje, govor, igru i upotrebu ruku.<br \/>Zato se u BDA pristupu polo\u017eaj glave ne posmatra izolovano. Dete se mo\u017eda vi\u0161e oslanja na jednu ruku, okre\u0107e se lak\u0161e na jednu stranu, sedi nagnuto ili ima vidljivu razliku u polo\u017eaju ramena i rebara.<br \/>Asimetrija ne mora uvek zna\u010diti problem, ali kada je izra\u017eena i ponavlja se u razli\u010ditim polo\u017eajima, ona mo\u017ee uticati na na\u010din na koji dete koristi telo. Ako je jedna strana grudnog ko\u0161a pokretljivija ili stabilnija od druge, dete mo\u017ee razviti obrasce kretanja koji se oslanjaju vi\u0161e na jednu stranu.<br \/>To mo\u017ee uticati na sedenje, okretanje, puzanje, hod, upotrebu ruku i ravnote\u017eu. Zato stru\u010dna procena ne posmatra samo da li dete mo\u017ee da izvede pokret, ve\u0107 i da li ga izvodi simetri\u010dno, efikasno i uz dobru organizaciju celog tela.<\/p><p><strong>Grudni ko\u0161, disanje i razvoj govora<br \/><\/strong><br \/>Disanje nije va\u017eno samo za posturu i pokret. Ono ima ulogu i u hranjenju, glasu, govoru i regulaciji aktivnosti. Stabilnost trupa i organizacija grudnog ko\u0161a mogu uticati na to koliko dete efikasno koristi dah tokom vokalizacije, izgovora ili du\u017eeg govora.<br \/>To ne zna\u010di da je svaki govorni izazov povezan sa grudnim ko\u0161em. Govor je slo\u017eena funkcija i zavisi od vi\u0161e sistema. Ipak, kod dece kod koje postoje motori\u010dke i posturalne pote\u0161ko\u0107e, korisno je posmatrati i osnovu iz koje govor nastaje: disanje, polo\u017eaj glave, stabilnost trupa i organizaciju usne duplje.<br \/>Zbog toga multidisciplinarni pristup \u010desto ima veliku vrednost.
